What Is 3D Guided Implant Placement?

Traditional dental X-rays provide useful two-dimensional information. A CBCT (Cone Beam Computed Tomography) scan provides a three-dimensional view of the teeth, jawbone and surrounding anatomical structures.

Using this information, the proposed implant position can be planned digitally before surgery.

When appropriate, a surgical guide can be produced based on this planning. The guide fits over the teeth or gums and helps guide the implant instruments according to the planned position.

Not every implant requires a surgical guide. The most appropriate technique depends on the individual case.

Why Is 3D Planning Important for Dental Implants?

A dental implant needs to be positioned with both the available bone and the final tooth position in mind.

3D planning can help your dentist assess:

• the height and width of available jawbone
• the proposed position and angle of the implant
• nearby teeth and tooth roots
• important anatomical structures
• whether bone grafting may be required
• the relationship between the implant and the planned crown or bridge

What Is a Surgical Implant Guide?

A surgical guide is a customised device made from the digital implant plan.

During implant surgery, the guide can assist with transferring the planned implant position, angle and depth to the mouth.

Guided implant surgery can be particularly useful when:

• implant positioning requires careful prosthetic planning
• several implants are being placed
• implants are being used to support a bridge
• there is limited available bone
• full-arch implant treatment is being planned
• the relationship to surrounding anatomical structures requires careful consideration

However, guided surgery is not automatically the best approach for every patient. In some situations, conventional implant placement provides better access or allows the dentist to adapt directly to the clinical findings during surgery.

Does Guided Implant Surgery Mean No Incisions or Stitches?

Not necessarily.

Some guided implant procedures can be performed using a small opening through the gum when the clinical conditions are suitable. Other cases still require the gum to be opened so the underlying bone can be properly assessed and managed.

Bone grafting or other additional procedures may also be required.

For this reason, we do not describe guided implant surgery as automatically being “flapless”, “stitch-free” or “pain-free”. The surgical approach is selected according to the individual patient’s anatomy and treatment requirements.

Can 3D Planning Avoid Bone Grafting?

3D planning can help determine whether there is sufficient bone for an implant and may assist in selecting an appropriate implant position.

However, it cannot create bone where adequate bone does not exist.

Some patients have sufficient bone for straightforward implant placement. Others may require a bone graft, socket graft, sinus lift or other procedure to provide suitable support for an implant.

2. CBCT Scan

A CBCT scan provides a three-dimensional assessment of the proposed implant area.

3. Digital Implant Planning

The CBCT and other clinical information are assessed to plan the proposed implant position and determine whether additional procedures may be required.

4. Surgical Guide if Appropriate

If guided implant placement is considered beneficial, a customised surgical guide can be prepared.

5. Implant Placement

The implant is placed according to the treatment plan. The exact surgical technique will depend on your individual circumstances.

Is 3D Guided Implant Placement Suitable for Me?

It may be used for anything from a single missing tooth to multiple implants or full-arch implant treatment.

Whether a surgical guide is worthwhile depends on factors including:
