What Is All-on-4?

All-on-4 is a treatment concept in which a fixed bridge replacing a complete arch of teeth is supported by dental implants.

In the traditional All-on-4 concept, four implants are positioned within the jaw to support the bridge. In suitable cases, the back implants may be angled to make use of available bone and provide appropriate support for the restoration.

The term has become familiar to patients and is often used more broadly when discussing fixed full-arch implant treatment.

Depending on the individual case, we may recommend four implants or consider additional implants.

Why Might We Use 4, 5 or 6 Implants?

There is no single implant number that is appropriate for every patient.

The decision depends on factors including:

• the amount and quality of available bone
• the size and shape of the jaw
• where implants can be appropriately positioned
• the patient’s bite and chewing forces
• the design of the final bridge
• whether any implants are being placed into grafted areas
• the condition of the gums and supporting tissues
• other individual clinical factors

Where anatomy and restorative planning allow, additional implants may provide another option for supporting and distributing forces across a full-arch bridge.

However, more implants are not automatically better. The number and position of implants should be determined by the overall treatment plan.

Who May Consider Full-Arch Dental Implants?

Full-arch implant treatment may be considered for patients who:

• have already lost all teeth in one or both jaws
• wear complete dentures and would prefer to explore a fixed option
• have multiple failing or severely compromised teeth
• have advanced dental disease where retaining the remaining teeth may not be predictable
• are looking for an alternative to a conventional removable full denture

A careful assessment is required before deciding whether full-arch implant treatment is appropriate.

Where natural teeth can reasonably be maintained, the advantages and disadvantages of retaining them should also be considered.

All-on-4 Versus Dentures

A conventional full denture is removable and rests primarily on the gums and underlying tissues.

A fixed full-arch implant bridge is attached to dental implants and is not removed by the patient for routine daily cleaning.

For suitable patients, implant-supported fixed teeth can provide greater stability than a conventional removable denture and may improve confidence when speaking and chewing.

However, this does not mean that dentures are unsuitable for everyone.

Conventional dentures remain a reasonable treatment option for many patients and involve less surgery and generally lower treatment costs. Some patients may also prefer an implant-retained removable denture, which uses implants to improve denture stability while allowing the denture to be removed for cleaning.

Do I Need Bone Grafting for All-on-4?

One advantage of careful full-arch implant planning is that implants can sometimes be positioned to make use of the patient’s existing bone.

This may reduce the need for extensive grafting in some cases.

However, All-on-4 does not guarantee that bone grafting can be avoided.

Some patients may require:

• bone grafting
• socket grafting following extractions
• sinus grafting or sinus lift procedures
• management of bone defects
• soft-tissue or gum grafting

Whether additional procedures are required depends on your anatomy and the planned implant positions.

A 3D CBCT scan allows us to assess the available bone and important anatomical structures before treatment.

Can Teeth Be Placed on the Same Day?

In suitable cases, a temporary fixed bridge may be connected to the implants soon after implant placement.

This is sometimes referred to as immediate loading or “teeth in a day”.

However, same-day fixed teeth are not appropriate or possible in every case.

Whether a temporary bridge can be attached immediately depends on factors such as:

• implant stability at surgery
• bone quality
• implant distribution
• the patient’s bite
• the extent of any additional surgery
• other individual risk factors

The decision is made according to the clinical findings rather than being guaranteed before surgery.

What Teeth Do I Have During Healing?

When immediate fixed teeth are suitable, patients generally receive a temporary full-arch bridge during the initial healing phase.

The temporary bridge is designed to provide appearance and function while the implants integrate with the surrounding bone.

Depending on the case, a fibre-reinforced temporary bridge may also be used as an interim or longer-term temporary restoration.

After healing and assessment, the definitive bridge can be planned and constructed.

The timing and type of final restoration depend on the individual treatment plan.

What Is the Treatment Process?

1. Consultation and Assessment

We discuss your concerns, dental history, treatment expectations and possible alternatives.

Your remaining teeth, gums, bite and general oral condition are assessed.

2. CBCT and Digital Planning

A CBCT scan provides three-dimensional information about the jawbone and surrounding anatomical structures.

3. Treatment Planning

We determine:

• whether remaining teeth should be retained or removed
• the proposed number and position of implants
• whether grafting or additional procedures may be required
• the proposed temporary teeth

4. Implant Surgery

Any teeth that require removal are managed according to the treatment plan, and the implants are placed.

Where clinically appropriate, a temporary fixed bridge may be attached during the initial treatment period.

5. Healing

The implants require time to integrate with the surrounding bone.

Patients with an immediate temporary bridge need to follow the dietary and cleaning instructions provided during this period.

6. Final Bridge

After appropriate healing and assessment, the definitive full-arch restoration can be completed according to the planned restorative pathway.

Can I Eat Normally With All-on-4 Teeth?

Full-arch implant-supported teeth are designed to improve chewing function compared with unstable or poorly fitting dentures.

However, patients should not assume they can immediately eat anything they want.

During the initial healing period, particularly when implants are immediately supporting a temporary bridge, a modified softer diet is important to limit excessive forces while the implants integrate.

After healing and completion of the definitive restoration, patients can generally progress to a broader diet according to their dentist’s advice.

Implant bridges are strong, but they are not indestructible. Very hard foods and excessive biting forces can damage restorative materials or components.

How Long Do All-on-4 Dental Implants Last?

There is no fixed lifespan that can be guaranteed for dental implants or an implant-supported bridge.

Long-term outcomes depend on factors including:

• oral hygiene
• gum and bone health
• smoking
• general health
• grinding or clenching
• biting forces
• implant position
• the design and material of the bridge
• regular professional maintenance

The implants and the bridge should also be considered separately.

An implant may remain integrated while components of the bridge require maintenance, repair or replacement over time.

For this reason, we avoid describing full-arch implant teeth as “permanent teeth for life”.

Cleaning and Maintaining Full-Arch Implant Teeth

Although the replacement teeth cannot develop decay, plaque and bacteria can accumulate around the implants and underneath the bridge.

Daily cleaning remains essential.

Depending on the bridge design, cleaning may involve:

• brushing
• interdental brushes
• bridge floss or floss threaders
• water flossers
• other cleaning aids recommended by your dental team

Regular professional reviews are also important so the implants, gums, bite and bridge can be monitored.

Is All-on-4 always four implants?

The traditional All-on-4 concept uses four implants. However, full-arch implant treatment can also use five or six implants depending on the anatomy, available bone and restorative treatment plan.

Are All-on-4 teeth removable?

A fixed full-arch implant bridge is secured to the implants and is not routinely removed by the patient. Your dentist may remove the bridge when clinically required for professional maintenance, repair or other treatment.

Can I have All-on-4 if I have bone loss?

Possibly. Bone loss does not automatically prevent implant treatment. A CBCT scan is used to assess the amount and location of available bone. Some patients can proceed using existing bone, while others may require grafting or a different treatment approach.

Can I avoid a denture completely?

Some suitable patients can receive a fixed temporary bridge around the time of implant placement. However, this cannot be guaranteed for every patient. We discuss the expected temporary restoration before treatment.

Are All-on-4 teeth permanent?

The bridge is fixed rather than removable by the patient, but “permanent” should not be interpreted as lasting for life without maintenance. Implant-supported restorations require ongoing cleaning, professional reviews and may require repair or replacement over time.

Can I eat normally after All-on-4 surgery?

A softer diet is generally required during the initial healing period. As healing progresses, your dentist will advise when and how to return to a broader diet.

Is All-on-4 my only option if I have no teeth?

No. Alternatives may include conventional full dentures, implant-retained removable dentures and different fixed implant configurations. The appropriate options depend on your anatomy, health, preferences and treatment objectives.
